Report form for reporting irregularities in the operation of Finance and IT Sp. z o.o.

The form allows for anonymous reporting. The Description of the problem field is the only required field in the form. However, it should be noted that voluntary provision of contact details by the sender of the report would enable notification of the sender about the way the matter is handled, as well as possible contact in a situation where additional information is needed for a thorough consideration of the report.

To enable a thorough investigation and appropriate response to the report, it should contain at least the following elements:

• the nature of the observed irregularity;
• description of the event;
• individuals involved in the reported irregularity;
• if possible, indicate documents that can confirm the irregularity.
